Child Welfare Complaint Form
Complaints about Colorado county child welfare practices are addressed by the Colorado Department of Human Services.
IMPORTANT:
- If you are concerned about a child or youth's safety and well-being, please call the Colorado Child Abuse and Neglect Hotline at 844-CO-4-KIDS (1-844-264-5437). Call 9-1-1 if there is an immediate threat.
- If you have a child welfare complaint, please contact your county human services department.
- If you want to appeal a finding, please contact the Child & Adult Mistreatment Dispute Review Section (CAMDRS) 303.866.4546.
- If you have contacted your county regarding your child welfare concern, but need further assistance from the state, please continue with filling out this form.
Please fill out this form as fully as possible. Complaints will be researched and a response will be sent to the complainant. Please note that the child welfare review process can take 6-8 weeks. You will receive a response in writing once the process is completed. If you choose to submit an anonymous complaint you may indicate that on the form. You will not be notified of any action taken as a result of this complaint.
